An identification program designed to provide vital information to a rescue team in an emergency situation.

Seniors who live alone, are alone for any certain period of time or are handicapped, are encouraged to complete a personal medical data form. This form is placed in a vial and secured in your refrigerator. An identification sticker is placed on the outside door which alerts the rescue squad to check for the vial. Vials and data forms are available at the Center without charge.
