Job Title: Recreation Program Coordinator- Part-Time Social Worker
Department: Recreation- Senior Center
Status: A. Classified B. Exempt
Job Summary:
Our vibrant senior center provides a place where active, independent senior citizens come for activities, exercise classes, and a hot lunch. The social worker will be responsible for assessing client needs and assisting seniors with individual and group meetings designed to educate and assist with aging issues. The goal is an improved quality of life. The social worker will work under the immediate supervision of the manager of Senior Programs.
Job Duties:
- Assist and advocate for seniors on a wide range of issues regarding their physical and mental health. Provide referrals as needed.
- Perform annual assessments and make home visits to homebound seniors to identify their needs and link them to services.
- Provide supportive counseling and referrals as needed.
- Assess clients in recognizing cognitive changes and provide intervention/referrals as needed.
- Assess for potential elder abuse and neglect and work with Adult Protective Services as required.
- Document services provided and client status according to program and agency guidelines.
- Plan and implement groups in the senior center covering various topics related to aging
Requirements:
- Bachelor’s degree in social work or closely related curricula;
- Licensed Independent Social Worker (LISW) registered to practice social work in the State of Ohio.
- Three years of social work experience with senior citizens preferred.
- Ability to express ideas and instructions clearly, and accurately and maintain confidentiality on departmental matters.
- Proficient with MSOffice
